Ability to add defects in the action page. When the work is reviewed, reviewer should be able to add defects in the action page. Hence the effort of action can be captured in the review action itself.

we are expanding defect capability to action as well.
configuration of 'Defect categories' and 'party at fault' will be come into builder app.

The functionality of defects is available only in the parent case, but the users should be able to add it in any action during the process.
